We regret to inform you of the death of

the late Jacek Kozakiewicz, MD, PhD

A graduate of the Medical University of Silesia in Katowice, born in 1975,

otolaryngology specialist.

Since 1989, Jacek Kozakiewicz has been associated with the Silesian Medical Self-Government. He served as the President of the Regional Medical Council in Katowice in 2009-2017. He was also Vice-President of the Supreme Medical Council (2018-2020) and Vice-President of the Regional Medical Council in Katowice (1993-1997 and 2018-2020).

In addition, for 31 years of work for the local government, he was m.in. Member of the Supreme Medical Council, Delegate to the National Congress of Physicians, Delegate to the Regional Congress of Physicians, Member of the Presidium of the Regional Medical Council in Katowice, Member of the Regional Medical Council in Katowice.

Jacek Kozakiewicz was also the originator and the first President of the Silesian Forum of Self-Governments of Public Trust Professions.

For his activities, he was awarded the Medal of the NRL “Salus aegroti suprema lex”, the Silver and Gold Medal of the Silesian Military Institute “Meritorious for Doctors PRO MEDICO”, the Gold Badge of Honour “For Merit for the Province of Silesia”. Silesia”, the Gold Medal in Defence of Your Rights, the Platinum Laurel of Skills and Competence “Pro Publico Bono”, the Gold Cross of Merit.

The funeral ceremony took place on Friday, July 5, 2024 at 10:30 a.m. in the Archcathedral of Christ the King in Katowice.
The burial took place in the cemetery of the parish of St. Peter and Paul at Sienkiewicza Street in Katowice.
